WTF is an up or down vote?

A friend writes:
What is all this bullshit about an "up or down" vote on all these people who need Senate confirmation? It's a vote. A vote by its very nature is up or down.
This is rhetoric served up by Karl Rove and spoon fed to everybody else to use, so that we'll all stay on message. Haven't you noticed the president, every Republican member of Congress, every cabinet member, and every talk show host, newspaper columnist, and TV gasbag is using that exact same phrase? Don't you think it's odd that after millennia of votes just being votes, everything is now an "up or down" vote? Have we all turned into such mindless robots that we all repeat exactly what we've heard on TV?
Um, don't answer that.
